Student Loan Debt Relief Application Preview

On October 11, 2022, a preview of the Student Loan Debt Relief Application Form was released by the Official White House's Twitter.
From the look of it, it is pretty straightforward and simple with no documents needed to be uploaded unless requested. A date has still not been given yet, however I assume it will be soon since they released a preview.
The application seems:
To be straightforward and simple
To be easily accessible on mobile devices and desktops
To not having to enter any log in information
To not having to upload and documents unless additional information is requested
The application seem to have three sections:
The first page gives an overview of the program including who's eligible
The second page, you'll enter your personal information. (No FASFA ID needed)
The third page, you'll self attest that you are indeed requesting relief, you meet income requirements, and provide proof of income if asked.
Once the simple application is completed, you'll then certify under penalty of perjury and submit.
